Liposuction and lipofilling complement each other well. The first procedure allows you to quickly get rid of fat deposits in unnecessary areas in one go, and the second, on the contrary — to model the desired areas, using your own fat as a filler.

More often for aesthetic purposes, these two procedures are performed simultaneously. In combination, the surgery is minimally traumatic, lasts from 30 minutes to 2 hours, has a short recovery period and long-lasting rejuvenating effect, and at the same time helps both women and men to forget about complexes, low self-esteem and insecurity in their look.

Liposuction and lipofiling can be carried out under general anesthesia, and under intravenous sedation with local anesthesia. It depends on the set of procedures performed at the same time.

Each face is unique and requires an individual approach to make it younger and more beautiful, to carry out a real modeling, not just remove fat. Therefore, I choose the most effective solution individually for each patient, depending on the specifics of the facial structure and the objectives set.

FACIAL LIPOSUCTION TECHNIQUES

TUMESCENT liposuction — Klein’s solution (a combination of saline, adrenaline, anesthetic) is injected into the treated area for 20-40 minutes. The composition softens the fatty tissue, which is then removed through medical cannulas (thin metal tubes) using a vacuum.

ULTRASOUND liposuction — Klein’s solution is injected under the skin, the effect of which is enhanced by ultrasound. Waves soften the fiber, while not affecting neighboring tissues. Fatty tissue is removed by cannulas with the help of a vacuum. The technique also contributes to the overall improvement of skin condition.

LASER liposuction — a laser beam acts on facial fat deposits, which turns solid cells into liquid cells. Destroyed fat deposits are removed from the body by the lymphatic and circulatory systems. The laser also triggers active regeneration processes and the formation of its own collagen. This shortens the recovery time and makes the skin firm.

RADIOFREQUENCY lipolifting — radio wave radiation penetrates into the deep layers and heats the cells to a temperature of 40-45 °C. There is a one-step destruction of fatty tissues and skin tightening, and active regeneration processes are triggered inside. After the procedure, there are no bursts or drooping chin.

In the course of liposuction, a significant mass of fat cells is removed, so the body needs some time to rebuild.

The final result can be judged only 6-12 months after the procedure. During the same period, if necessary, corrections can be made, although in most cases the desired result is achieved after one procedure.

Most patients whose expectations were realistic are very satisfied with the results of liposuction.

FACIAL LIPOFILLING TECHNIQUES

MACROLIPOFILLING — relatively large fragments of adipose tissue are used, which makes it possible to restore or create volume to fill the desired areas: temporal area, cheekbones, cheeks, chin. Fat is harvested using a blunt-tipped cannula with a hole diameter of approximately 2 mm. After cleaning, the fat is injected subcutaneously, intermuscularly, intramuscularly using a cannula with a diameter of about 1.2 mm.

MICROLIPOFILLING is used in areas with very thin or no fat layer: around the eyes, neck, lips, etc. For the intake of smaller droplets of fat used cannulas with a diameter of holes no larger than 1.0 mm, and for subcutaneous injection — diameter 0.7-0.9 mm. Also “microfat” can fill wrinkles, nasolabial and nasolacrimal furrows, correct skin changes after acne, shallow scars, stretch marks, etc. The fat is injected subcutaneously and intradermally with a very small diameter sharp needle (SNIF method — Sharp Needle Intradermal Fatgrafting).

NANOPHETGRAFTING — the procedure uses fat that has undergone multi-stage preparation with syringes and a special nylon mesh of very tight weave, and then centrifuged. The result is a “nanofat” — a small amount of pinkish liquid rich in stem cells. This fluid is injected with thin needles intradermally to correct dark circles under the eyes, on the slopes of the nose, fine facial wrinkles, etc.

Often patients before the surgery are in fear of future changes in their appearance and ask not to inject “extra fat”, but to limit themselves to a very small amount.

We discuss the planned volume of subcutaneous fat transplantation, taking into account all your individual characteristics.

For example, lipofilling cheekbones involves the introduction of 6 to 14 ml of fat. If you stop at the minimum volume of 6 ml, then by the third month after the procedure at best 5 ml will remain, or even 3.5 ml. This is not enough for the desired effect.

The protocol of procedures differs depending on the chosen technique, but in general includes the following steps:
  • markings are made on the skin of the face with a cosmetic marker, according to which incisions and punctures are made with a length of no more than 3 mm;
  • in the designated areas are introduced cannulas of minimal diameter (only 1.5-2 mm), which do not traumatize the skin and do not leave postoperative scars;
  • is introduced tumescent solution to melt fatty tissues;
  • is carried out impact vacuum, ultrasound, laser or radio wave beam (depending on the chosen methodology);
  • through cannulas carefully removed excess fatty tissue and formed a new relief;
  • cosmetic sutures and aseptic dressing are applied.
Modern technology and my experience allow me to return youth and beauty with minimal traumatization of tissues. Liposuction and lipofilling are suitable for most patients regardless of gender and age.
Surgery will have to be waived in the following cases:
  • Psychiatric disorders, including borderline personality disorder;
  • exacerbation and decompensation of severe somatic pathologies;
  • uncertainty about the need for surgery or unrealistic expectations;
  • acute and chronic diseases of internal organs;
  • oncological diseases of any localization;
  • autoimmune diseases;
  • dermatologic diseases (inflammatory, infectious, fungal);
  • herpes-virus infection in the stage of exacerbation;
  • presence of acute inflammatory or infectious processes, acute respiratory viral infections;
  • acute or decompensated diseases of the cardiovascular system, kidneys, liver, diabetes mellitus;
  • trauma in the area of surgery;
  • congenital hemophilia or the state after taking anticoagulants (blood clotting disorders);
  • endocrine disorders;
  • severe deviations from the normal shape of the face, requiring complete facial plastic surgery;
  • intolerance to anesthesia due to severe comorbidities, advanced age;
  • the presence of allergic rashes, redness, etc.;
  • period of pregnancy and lactation.
In order to shorten and ease the rehabilitation period, you need to prepare for the surgery thoroughly and correctly. This will help you “get back on track” faster.
BEFORE SURGERY
  • follow all my recommendations during the preparation phase, that’s the most important thing;
  • the key to the success of the surgery is your health condition and optimistic attitude;
  • do not hesitate to ask clarifying questions about the processes and stages of the surgery, which you do not understand or cause fear, it will add confidence in the successful result;
  • postpone surgery even if you have minor symptoms of acute respiratory infections until you are fully recovered;
  • smoking and excessive alcohol consumption can impair your health and affect the rehabilitation process;
  • if you are prone to bleeding or bruising, tell the surgeon and anesthesiologist;
  • one week before the surgery exclude the intake of all dietary supplements, vitamins;
  • inform in advance about taking any medications, oral contraceptives.

The most important stage of preparation for any surgery is the preliminary diagnostic. Only after passing the mandatory tests and examinations, which will determine the presence or absence of contraindications to surgery, we will be able to discuss with you the date of surgery, or its replacement by other procedures.

Diagnostic examinations:
  • ECG with a cardiologist’s report;
  • fluorography or chest X-ray;
  • in the presence of concomitant diseases — the conclusion of a specialized doctor about the absence of contraindications to surgical intervention;
  • An orthodontist may need to be consulted based on the results of the examination.
Labolatory tests:
  • general blood test (not older than 10 days);
  • urinalysis (not older than 10 days);
  • coagulogram (ACTH, prothrombin, fibrinogen, not older than 10 days);
  • blood group, Rh factor;
  • RW (Wasserman reaction), HIV, Hepatitis B and C (HBS, HCV), (not older than 3 months);
  • biochemical blood test (AlAT, AsAT, total protein, glucose, total bilirubin, creatinine, urea, electrolytes (K, Na), (not older than 10 days).

If minor abnormalities are detected, such as in biochemistry, the surgery is still performed, but after prior medication to normalize the indicators. If there are unforeseen, temporary or other health problems, the surgery should be postponed for your safety. Please do not forget to mention any “minor” health problems you may have.

For me, your health and safety are paramount, which is why I perform the surgery only when I am 100% sure that there are no contraindications and I know your full medical history.

10 days before the scheduled date of surgery, I recommend completely exclude alcohol and 3 days — smoking cigarettes, which worsen healing and can provoke complications, as well as negatively affect the final result of the surgery.

Cosmetic procedures that aggressively affect the skin (chemical or deep peeling) are not recommended 14 days before the surgery, and 7 days before — visiting a solarium. If possible, eliminate inflammation or irritation on the face.

Also for 14 days you should temporarily suspend the use of anticoagulants (aspirin, heparin, vitamin C, cardiomagnil, thrombo-ACS, painkillers and antipyretics) and hormonal drugs, including contraception. If it is impossible to refuse any medication, be sure to inform me about it at the first consultation, so that I can give recommendations on how best to proceed in such a case: to exclude the drug, replace it or leave it as it is.

The surgery is performed on an empty stomach, so on the eve of the surgery after 8 pm nothing can be eaten! After 12 at night do not drink, even just water! From coffee and energy drinks I advise you to give up at least a couple of days before the surgery.

Menstrual cycle does not affect the surgery, I recommend my patients to plan the date of surgery on the 6th-8th day from the beginning of the cycle. If menstruation occurs earlier than expected, be sure to inform me and the anesthesiologist.

ON THE DAY OF SURGERY
  • When going to the clinic for surgery, do not forget your passport and all tests in printed form (make copies in advance if you need a second copy).
  • Do not use perfume, cosmetics and any means with a pronounced fragrance. Leave jewelry, valuables and money at home. You will definitely not need them at the surgery 😉
  • Remove everything from the nail of the forefinger on your left hand, including the false nail. This is necessary for attaching the pulse oximeter (a device to monitor blood oxygen saturation).
  • Choose comfortable clothing that you will not have to take off and put on “over your head”. Clothing with zippers or buttons is best.
  • If you have varicose veins, you will need compression stockings.
Take care in advance how you will get home from the clinic, because you will not be able to drive after the surgery.
Thanks to modern equipment and the latest technology, my patients do not experience significant discomfort after the procedures. Hematomas and swelling will go away quickly if you follow all my recommendations.
POST-SURGERY REHABILITATION
  • The speed of recovery depends on the individual characteristics of the patient: the ability of tissues to regenerate, tendency to edema and hematoma formation, pain threshold, age, the presence of bad habits, previous surgeries and aggressive manipulations in the field of fat grafting.
  • During the rehabilitation period you will need a calm environment, support and understanding of close people. The formation of a neat postoperative scar depends largely on you and on compliance with all my recommendations.
  • The first day after plastic surgery should remain in the hospital clinic under the supervision of medical staff. This is required to exclude the risks of possible complications and negative reactions of the body. If after 24 hours you feel well and have no complaints, you can return home to normal life, but with postoperative restrictions.
  • For the first 3 weeks after surgery, you can sleep only on your back with an elevated headboard.
  • Baths, saunas, climate change to a hotter one, physical activity should be completely excluded for 3 months after surgery.
  • Try to keep your weight at the same level.
POST-LIPOSUCTION
  • Active recovery lasts about 2 weeks. During this time it is necessary to wear a compression bandage, which helps to reduce swelling and prevents deformation of the skin.
  • The final result you will see in 1 — 3 months, when the tissues are finally restored, the contours of the face will become clearer, and the skin — firm and smooth.
  • In addition, to accelerate tissue recovery can be carried out lymphatic drainage, physiotherapy, microcurrents and other procedures.
POST-LIPOFILLING
  • Sometimes there is only a slight, so-called “youthful” edema and small hematomas that disappear within 5-7 days.
  • Before full recovery from 10 to 30% of the transplanted adipose tissue is lost. This is a normal phenomenon. Moreover, the more patients are prone to fullness, the greater the percentage of fat takes root. Thin patients more often need 2-3 lipofilling procedures to achieve the desired result.
  • The transplanted fat should not be kneaded or crushed for the first 3 weeks after surgery. If the irregularities are persistent, perform gentle finger massage 3-4 times a day, but only after consulting with me!
  • In your daily diet should be foods containing polyunsaturated fats: olive oil, walnuts, red fish, sour cream, cream, fatty cottage cheese, etc.
  • For three months after surgery, you must not lose weight! If the body weight decreases, the transplanted fat will dissolve first!

It is important to take into account!
Postoperative “youthful” edema starts to build up over time and the volume of the affected areas of the face can increase 2-4 times! No matter how much you like this effect, you do not need to get used to the new shape of the face, because by the third month of rehabilitation swelling begins to pass and remains exactly the volume of fat that was originally planned (taking into account the partial “melting”). It is this volume will remain with you for a long time and will naturally improve and lose weight along with the rest of the tissues. Therefore, in 3-6 months after lipofilling, some patients return for a “supplement” and ask to return the “rejuvenating” swelling.

MEDICINAL PURPOSES
  • Traumel C cream — thanks to its antiseptic and regenerating properties, it is suitable for skin care after traumatization. Restores a healthy complexion, soothing the skin.
  • Bepanten cream — helps to strengthen the skin’s protective barrier that retains moisture, reduces irritation, redness and accelerates the healing of superficial skin damage.
  • Elocom cream — hormone (glucocorticosteroid) has anti-inflammatory, antipruritic, antiexudative effect. The drug reduces redness and swelling, leading to vasoconstriction at the site of inflammation. Duration of use is no more than 3-5 days.
  • Lyoton gel — has antithrombotic, anti-edematous and anti-inflammatory effect, improves blood microcirculation and activates tissue metabolism, which accelerates the resorption of hematomas, reduces swelling of tissues.
