The quickest way to make the face slimmer and the cheekbones higher and more expressive is to remove “Bisha lumps” (fatty lumps inside the cheeks). As an additional effect, the face is significantly rejuvenated. In women, such plastic surgery lightens the lower third of the face, in men — corrects age-related rounding of the cheeks.
“Bisha lumps” removal is performed by conventional and endoscopic methods.
In the first case, 1-2 cm incisions are made on the inside of both cheeks. The fat lump is removed through this incision. After that, I carefully check the cheeks for symmetry and then on the inside of the cheeks I apply self-absorbing sutures.
The endoscopic method is performed if “Bisha lumps” removal is part of a plastic surgery procedure. In some cases, to achieve a more graceful facial shape, “Bisha lumps” removal alone is not enough and I recommend combining the procedure with liposuction.
The amount of “Bisha lumps” to be removed in each case is determined individually depending on the shape of the face and the desired result. In certain cases, the fat clumps are not removed, but moved to the cheekbone area to give them a more pronounced shape and create additional volume.
With local anesthesia, the surgery takes 25-30 minutes. In individual cases, general anesthesia may be used. The method of anesthesia is determined at the first consultation and depends on the complexity of the surgery, your wishes and the presence / absence of contraindications to general anesthesia.
Removal of “Bisha lumps” as a method of cheekbone correction is not suitable for all patients. This is due to the peculiarities of the facial structure. Sometimes alternative non-surgical procedures are much more promising.
The surgery will have to be abandoned in the following cases:
- Under 25 years of age (facial tissues are still forming, the amount of fatty tissue may change);
- desire to reduce or increase your weight in the near future (when losing weight, fatty lumps will “melt” first);
- acute infectious and inflammatory diseases;
- respiratory infections in acute form;
- blood diseases and clotting disorders;
- deterioration of immunity and the emergence of systemic diseases in chronic form;
- severe neurological and somatic diseases;
- mental disorders;
- chronic liver disease;
- the appearance of inflammation in the oral cavity, the skin of the face and neck;
- exacerbation of diabetes mellitus;
- malignant formations;
- tuberculosis, HIV, viral hepatitis;
- pregnancy and lactation.
To correct the lower part of the facial oval, create a beautiful “hollowing” of the cheeks and model the cheekbones, I recommend removing or relocating “Bisha lumps”.
BEFORE SURGERY
- follow all my recommendations during the preparation phase, that’s the most important thing;
- the key to the success of the surgery is your health condition and optimistic attitude;
- do not hesitate to ask clarifying questions about the processes and stages of the surgery, which you do not understand or cause fear, it will add confidence in the successful result;
- postpone surgery even if you have minor symptoms of acute respiratory infections until you are fully recovered;
- smoking and excessive alcohol consumption can impair your health and affect the rehabilitation process;
- if you are prone to bleeding or bruising — tell me and the anesthesiologist;
- one week before the surgery exclude all supplements and vitamins;
- inform me and the anesthesiologist in advance if you are taking any medications or oral contraceptives.
Presurgery preparation includes standard medical examination. In case of chronic diseases, additional diagnostics and consultation of specialized specialists (endocrinologist, cardiologist, gynecologist) may be prescribed.
At the initial consultation, I conduct an examination and assess the feasibility of surgery. If the removal of fat cells will bring you closer to the desired result, the volume of fatty tissue to be removed is discussed.
Diagnostic examinations:
- ECG with a cardiologist’s report;
- fluorography or chest X-ray;
- in the presence of concomitant diseases — the conclusion of a specialized doctor about the absence of contraindications to surgical treatment.
Laboratory tests:
- general blood test (not older than 10 days);
- urinalysis (not older than 10 days);
- coagulogram (ACTH, prothrombin, fibrinogen, not older than 10 days);
- blood group, Rh factor;
- RW (Wasserman reaction), HIV, Hepatitis B and C (HBS, HCV), (not older than 3 months);
- biochemical blood test (AlAT, AsAT, total protein, glucose, total bilirubin, creatinine, urea, electrolytes (K, Na), (not older than 10 days).
If minor abnormalities are detected, for example, in biochemistry, the surgery is still performed, but after prior medication preparation to normalize the indicators. If there are unforeseen, temporary or other health problems, the surgery should be postponed for your safety. Please do not forget to mention any “minor” health problems you may have.
For me, your health and safety are paramount, which is why I perform the surgery only when I am 100% sure that there are no contraindications and I know your full medical history.
10 days before the scheduled date of surgery, I recommend completely exclude alcohol and 3 days — smoking cigarettes, which worsen healing and can provoke complications, as well as negatively affect the final result of the surgery.
Cosmetic procedures that aggressively affect the skin (chemical or deep peeling) are not recommended 14 days before the surgery, and 7 days before — visiting a solarium. If possible, eliminate inflammation or irritation on the face.
Also for 14 days you should temporarily suspend the use of anticoagulants (aspirin, heparin, vitamin C, cardiomagnil, thrombo-ACS, painkillers and antipyretics) and hormonal drugs, including contraception. If it is impossible to refuse any medication, be sure to inform me about it at the first consultation, so that I can give recommendations on how best to proceed in such a case: to exclude the drug, replace it or leave it as it is.
The surgery is performed on an empty stomach, so on the eve of the surgery after 8 pm nothing can be eaten! After 12 at night do not drink, even just water! From coffee and energy drinks I advise you to give up at least a couple of days before the surgery.
Menstrual cycle does not affect the surgery, I recommend my patients to plan the date of surgery on the 6th-8th day from the beginning of the cycle. If menstruation occurs earlier than expected, be sure to inform me and the anesthesiologist.
ON THE DAY OF SURGERY
- When going to the clinic for surgery, do not forget your passport and all tests in printed form (make copies in advance if you need a second copy).
- Do not use perfume, cosmetics and any means with a pronounced fragrance. Leave jewelry, valuables and money at home. You will definitely not need them at the surgery 😉
- Remove everything from the nail of the forefinger on your left hand, including the false nail. This is necessary for attaching the pulse oximeter (a device to monitor blood oxygen saturation).
- Choose comfortable clothing that you will not have to take off and put on “over your head”. Clothing with zippers or buttons is best.
- If you have varicose veins, you will need compression stockings.
Take care in advance how you will get home from the clinic, because you will not be able to drive after the surgery.
Bichectomy is an uncomplicated surgery, no hospital stay is required. And to avoid inflammation in the incision area, strictly follow my recommendations.
POST-SURGERY REHABILITATION
On average, the oral mucosa heals in 2-3 days, swelling and soreness may persist for about 5 days, the full feeling of discomfort usually passes on the 14th day after surgery.
Until full recovery can not do sports, other physical activity should be reduced as much as possible. Try to sleep on your back.
Avoid visiting baths and saunas. Also for the time of recovery postpone cosmetic procedures, facial massage and visits to the dentist.
Also in the first couple of weeks restrain mimic activity. During this period, healthy laughter is contraindicated for you. Replace it for a while with “smile of Joconde” 🙂
POSSIBLE COMPLICATIONS
The mucous tissues heal quickly, so serious complications are extremely rare, only if you ignored the rules of the rehabilitation period or hid any information from me about the state of your health.
Post-surgery swelling may persist for 10 days, it is a natural reaction of the body to the intervention. And in the first 2-3 days after the surgery because of the strong swelling of the cheeks may seem even wider than before the surgery. Do not worry. At most three weeks swelling will go down and you will see the preliminary result, which will surely please you.
DIET AFTER “BISHA LUMPS” REMOVAL
The postoperative diet should consist exclusively of liquid food for the first three days, then for the next few weeks you will have to avoid solid food, which requires effort to chew.
The temperature of the food taken should be medium, too hot or, conversely, cold food is not allowed. Every time after a meal, even if you just ate a piece of apple, you should brush your teeth or, if there is no such possibility, rinse your mouth thoroughly.
Evaluate the results of the intervention can be assessed after a few weeks after the swelling subsides. The final appearance of the face “will acquire” in about six months, when there will be complete healing and deposition of tissues.
